PLAYER PRE-REGISTRATION
Each player will need to complete the online pre-registration form by clicking on the following link:

If you have any problems with the form or need to make any changes after you have submitted your form, please contact the U.S. Open Player Services office at 908.748.4770. Please note: In addition to completing the online player pre-registration form, each player must go through player registration on-site at the championship.

PRIZE MONEY FORM INFORMATION

For Professionals:
Each professional player will need to complete the following Prize Money Form prior to the championship. *For any questions pertaining to the prize money form, please contact Josh Prugar at jprugar@usga.org or 908.326.1852.
